Working Papers Application Procedure

1. The application for working papers can be picked up in the Guidance Office or by clicking on the link below. Please complete Part I only.

2. A copy of a physical examination is needed to acquire working papers. This exam must have been given within 12 months prior to the issuance of the working papers. If the school nurse has an updated copy of the physical examination on file, she is able to make a copy for you.

3. The application and physical can be submitted to Mrs. Dejuri in the Guidance Office, or by emailing dejurik@kpcsd.org. Please call 631-269-3349 with any questions.
